Supportive Home Care

Practical, everyday support in the home and community, provided according to the member's authorization, service plan, and our staffing.

  • Assistance and cueing with activities of daily living and household routines
  • Supervision, safety monitoring, and reminders
  • Companionship and social engagement

  • Essential housekeeping, laundry, and chore support

Transportation and community accompaniment are provided only when separately authorized and accepted.

Personal Care Services

Hands-on personal care in the member's home, provided under an individualized plan of care. Support includes:

  • Bathing, hygiene, and skin care (excluding wound care)
  • Dressing and grooming
  • Toileting and continence care
  • Eating and feeding assistance
  • Mobility, ambulation, and transfers

Naviah Care Services LLC is currently accepting personal care clients. Wisconsin Medicaid billing for personal care services is not yet available.

What is Supportive Home Care?
Supportive Home Care is day-to-day, non-medical support in the member's home and community: help and cueing with daily routines, supervision and safety monitoring, companionship, meal preparation, essential housekeeping and laundry, shopping and errands, respite for family caregivers, and community access when authorized. Services follow the member's authorization and individualized service plan.
What are personal care services, and how are they paid for?
Personal care is hands-on assistance with bathing, hygiene, dressing, grooming, toileting, eating, mobility, and transfers. Naviah Care Services LLC is currently accepting personal care clients. Wisconsin Medicaid billing for personal care services is not yet available, so personal care is arranged through private pay or another payment source you arrange. We do not claim participation with any insurer or payer unless it is expressly listed.
How do I request care or make a referral?
Use the referral form on this site or call our office at 262-222-9663. Family members, guardians, case managers, care managers, social workers, and hospital or facility discharge planners can all submit a referral.
Does submitting a referral guarantee services?
No. A referral is an inquiry. Services depend on assessment, written authorization or service plan, staffing and worker competency, and payer or program requirements. Submitting a referral does not confirm payer participation, authorization, acceptance, or a start of care.
